Erythema nodosum er trolig en form for overfølsomhetsreaksjon eller allergisk reaksjon i huden som opptrer på grunn av en annen lidelse. Vanligvis (opptil 55 prosent) finner vi ikke den underliggende årsaken til erythema nodosum, men det er mange mulige årsaker. Tidligere var tilstanden et velkjent tegn på tuberkulose, men i dag er andre årsaker vanligere. Streptokokkinfeksjon (halsbetennelse) er i dag den vanligste, kjente utløsende faktoren (cirka 30-50 prosent), særlig hos barn. Andre typer infeksjoner kan også utløse erythema nodosum: Medikamenter (tre til ti prosent) - for eksempel p-piller - og hormonelle reaksjoner, kronisk betennelsessykdom i tarmen (en til fire prosent) og sarkoidose (opptil 25 prosent) er andre vanlige årsaker hos voksne. Kreftsykdommer gir sjelden erythema nodosum.
