Diabetes og ikke-alkoholisk fettleversykdom
Dette er en leversykdom som ligner alkoholisk leverskade, men der det ikke foreligger høyt alkoholinntak. Årsaken er ukjent, men sykdommen er ofte forbundet med type 2 diabetes og fedme. Tilstanden kan utvikle seg videre og føre til betennelse i leveren (steatohepatitt) og i noen tilfeller til skrumplever (cirrhose). Alle pasienter som har alvorlig fedme og som har diabetes, har noen grad av steatose og der halvparten har steatohepatitt.
Diagnosen stilles på grunnlag av vedvarende forhøyede leverprøver (transaminaser). Ytterligere blodprøver bør tas, samt at det bør gjøres ultralyd og CT.
De fleste pasientene er uten symptomer. Noen kan oppleve illebefinnende eller oppfylhet i øvre høyre del av buken, men det er uklart om dette skyldes sykdommen. Hos pasienter med symptomer kan sykdomsbildet variere fra det bagatellmessige og til alvorlig leversykdom.
Gradvis vekttap og god blodsukkerkontroll anbefales for pasienter med ikke-alkoholisk steatohepatitt. Medikamentell behandling med metformin synes å kunne bedre tilstanden, i hvert fall på kort sikt.
