Utseende er typisk ved urtikaria, og legen kan som oftest lett se hva dette er. Problemet kan være å finne årsaken.

Ved akutt urtikaria gjøres vanligvis ingen bestemte undersøkelser, med mindre man har mistanke om at urtikariaen skyldes en utløsende sykdom som krever videre utredning. Det er imidlertid viktig å utelukke anafylaksi, allergisk sjokk, som krever rask behandling.

Dersom legen mistenker at tilstanden skyldes fysikalsk urtikaria, kan man forsøke å provosere fram vabler ved å utsette huden for kulde, vann eller risp. Ved kronisk urtikaria som ikke utløses av slike faktorer, vil legen undersøke videre for å finne en annen mulig årsak. Dette kan innebære undersøkelser av indre organer.
