Den vanligste plagen knyttet til Chiari malformasjon er hodepine, og som regel er denne hodepinen lokalisert til bakhodet. Hosting eller det å bøye hodet bakover kan forverre hodepinen.

Ensidig svakhet, særlig for håndgrep, er blant de mindre vanlige symptomene tilstanden kan gi. Ustøhet og halvsidig tap av temperatur- og smertesans i bein/arm forekommer også.
