Jordmor eller lege kan få mistanke om tilstanden allerede på nyfødt- eller barselavdelingen, men en studie viste at kun 7% av barna ble diagnostisert ved fødselen, og ved 5 års alder hadde 63% fått diagnosen.

Noen ganger er det helsesøster som reagerer. Det hender også at foreldrene får mistanke om at noe er galt, basert på forsinket utvikling eller symptomer fra sentralnervesystemet. Barnet viser ofte redusert interesse for sine omgivelser, og barnet når ikke milepælene som forventet.
