Kronisk leggsår

Kronisk leggsår er sår nedenfor kneet, gjerne i ankelregionen, som ikke gror i løpet av seks uker. Årsaken er svekket drenering av venøst blod fra leggen, for eksempel ved åreknuter.

Hva er kronisk leggsår?

Kronisk leggsår er sår nedenfor kneet, gjerne i ankelregionen, som ikke gror i løpet av seks uker. De første tegnene er ofte at huden på leggen blir tynnere og etter hvert misfarget. Det kan også forekomme eksem med kløe. Såret kan oppstå spontant eller etter en skade, og det har følgende typiske trekk:

  • Det er som regel lokalisert på innsiden av nedre tredel av leggen
  • Såret er fra få millimeter til flere centimeter i lengde og bredde, og det har skrånende kanter
  • Sårbunnen er klebrig eller væskende, og den er belagt med gulhvitt og dødt materiale.
  • Smertene er ofte beskjedne

Forekomsten av leggsår i befolkningen er på mellom 0,1-1 prosent. Blant personer over 65 år er forekomsten tre til fire prosent. Eldre kvinner er mest utsatt. 70-80 prosent av alle leggsår skyldes svikt i samleåresystemet (venøse leggsår). Resten er en kombinasjon av venøse og arterielle sår forårsaket av diabetes type 1 og type 2 eller røyking. Disse risikotilstandene medfører i tillegg til venesvikt innsnevringer i de små pulsårene (arterielle sår). Rene arterielle sår er lokalisert nederst på fotryggen og på tærne.
