Diagnosen stilles ved å påvise forstørret skjoldkjertel med en eller flere knuter i kjertelen.

For å kartlegge stoffskiftet analyseres TSH og fritt-T4, og man kan også i blodprøver finne ut om det foreligger en autoimmun betennelse i skjoldkjertelen. Ved hjelp av disse prøvene finner man ut om stoffskiftet er normalt.

Når det påvises knuter i skjoldkjertelen bør knutene undersøkes med ultralyd hos spesialist i. Ved ultralyd kan en ekspert ofte skille godarta fra ondarta kuler. Dersom kulene ikke er sikkert godarta, tas det celleprøve (finnålscytologi) hvor man kan identifisere de ondarta knutene. 

Dersom det påvises kreftforandringer vil det også bli foretatt ytterligere utredning med CT eller MR av halsen for å kartlegge situasjonen før behandling.
