Tuberkulose, forebygging: BCG vaksine

Vaksinasjon med BCG (bacille Calmette-Guérin) stimulerer cellulær immunitet. Det er usikkert om BCG-vaksinen forebygger lungetuberkulose. BCG vaksinens beskyttende effekt på helsearbeidere er heller ikke godt dokumentert. Varigheten av BCG vaksinens eventuelt beskyttende effekt er ikke klarlagt. Fra skoleåret 2009/2010 er BCG-vaksine ikke lenger en del av barnevaksinasjonsprogrammet i Norge, men det er erstattet med målrettet vaksine til risikogrupper.

Alle nyfødte, friske barn av foreldre som kommer fra land med mye tuberkulose, anbefales vaksinert. Vaksinen settes på venstre overarm ved 6-ukers kontroll på helsestasjonen. Etter 10-14 dager kommer det en hudreaksjon som etter hvert utvikles til et sår som væsker litt. Deretter dannes det en skorpe og såret tørker inn i løpet av fire til åtte uker. Det vil bli et arr som skrumper og blekner over tid.

Andre grupper som anbefales vaksine:

  • Personer som skal oppholde seg i land med særlig høy forekomst av tuberkulose i mer enn tre måneder, og ha tett kontakt med lokalbefolkningen.
  • Helsepersonell og helsefagstudenter som skal ha direkte pasientkontakt eller arbeide i laboratorier som driver tubekulosediagnostikk.
  • Andre arbeidstakere med særskilt risiko for tuberkulosesmitte (bistandsarbeidere, helsetjeneste, fengselsomsorg o.l. i land med høy forekomst av tuberkulose).
  • Yngre voksne som vurderes å ha særlig økt risiko for smitte
